Abstract: The lead article, Health Care: Whose Responsibility?, by Marcus H. Samo, traces the shift from trust territory days, when health care was heavily subsidized and widely available at little cost, to present challenges of financing and responsibility. It asks whether governments, communities, or families should be accountable for the growing costs of medical services in the FSM.

Additional sections describe shortages of medicine and staff in dispensaries in Pohnpei and Chuuk, difficulties maintaining hospitals in Kosrae and Yap, and public expectations of continued U.S. assistance. Comparisons with referral care in Guam and Hawai‘i highlight the widening gap between local resources and patient needs.
